| Report : Crop Circles |
| 1. Provide your name...This is not necesary if you wish to remain anonymous 2. Give us your e-mail address and a contact number if you wish to do so. 3. Tell us the address or location of the crop circle. 4. Tell how you found the crop circle. 5. State how many circles are at the location. 6. Let us know if you have seen any strange lights or any unusual activity in the sky or on the ground in the the area of the circles. 7. Tell us the date that you first noticed the Crop Circles. 8. Let us know if you or someone else has entered the Crop Circles by foot, 4 wheeler, vehicle, ect. 9. Please give us a time that we can set up an interview with you if you wish to do so. This can be by phone or in person. |
