| Please provide the following information with your e-mail and we will respond as quickley as possible to investigate your report! |
| 1. Please provide your name, address, phone number, and e-mail address Note: This is not necessary if you wish to remain anonymous. 2. Give the address or location that the animal or creature was spotted. 3. To the best of your ability give a description of the unexplained animal. 4. Was the creature or animal witnessed by more then one person? If so provide their contact information also. 5. Was the creature seen at night or during the day? 6. Were there any visible signs or tracks left by the creature? 7. How many times have you seen the creature? 8. What time did the sighting take place? 9. Were there any strange smells associated with the animal? 10. Was there more then one creature spotted? 11. How long was the creature or animal in site? |
