| Please provide the following information and we will respond as quickly as possible to investigate your report! |
| 1. Please give your name, address, telephone number, and e-mail address. NOTE: This is not necesary if you wish to remain anonymous. 2. Give the address of location that is haunted or the place where paranormal activity in occuring. 3. Describe the paranormal activity to the best of your ability. 4. Do the events occour frequently or just happen on occasions? 5. What time do the paranormal events usualy happen? 6. Have you experianced any strange sounds, smells, cold spots, drafts, voices, or anything else out of the ordinary at the location being reported? 7. Have you seen a ghost or spirit in a human like form at the location? 8. Does the activity scare you? 9. Do you have any pictures of paranormal anomalies at the location being reported? 10. If you answered yes to question 9. then please provide the pictures with your e-mail if possible. 11. Please give us a time to set up an interview with you if you wish to do so. This can be in person or by phone. |
